The problem is that when surgery looms as a possibility, one answer he gives is worth seventy-five dollars to him (or whatever he gets for that office visit) but the other is worth five thousand dollars (or whatever he gets for doing the surgery). If his medical belief system is geared toward justifying the incorporation of surgery into the game plan, it goes a long way toward helping him overcome his rather staggering economic commitments to Canada Mortgage Company.
